If you're a trans or gender-diverse adult in Tennessee, you've probably been watching the news and trying to figure out what it actually means for you. There's a lot of noise, a lot of fear, and not enough straightforward answers. Gender-affirming hormone therapy is still legal for adults in Tennessee, and affirming providers are still here accepting new patients and committed to the communities this state has been trying to push out. This post is meant to give you the information you need and to make clear where Blue Sage Wellness stands.
What the Law Actually Says in Tennessee
It's worth being clear about what has changed and what hasn't because fear is filling in the gaps where information should be.
In 2023, Tennessee enacted SB1, which bans gender-affirming care, including hormone therapy, puberty blockers, and surgery, for people under 18. In June 2025, the U.S. Supreme Court upheld that ban in United States v. Skrmetti, ruling it constitutional. That law applies to minors. Gender-affirming hormone therapy for adults remains legal in Tennessee.
In February 2026, Vanderbilt University Medical Center announced it was discontinuing gender-affirming plastic surgeries for adults. That is a provider access issue, not a legal one. Major health systems are making institutional decisions shaped by political pressure. The law has not changed for adults.
A bill has been introduced in the Tennessee legislature that would ban TennCare from covering GAHT for adults. As of this writing, that bill has not passed. We will update this post if that changes. What we can tell you right now is that adult access to gender-affirming hormone therapy in Tennessee is legally intact, and we are here providing it.

We don't think it's useful to treat the current climate as neutral because it isn't. The sustained legislative targeting of trans people, especially trans youth, has real mental health consequences that don't disappear the moment someone turns 18. Research consistently shows that when trans people lack access to affirming care, rates of depression, anxiety, and suicidal ideation increase significantly. Access to GAHT is associated with dramatically improved mental health outcomes across every measure.
Trans people of color, particularly Black trans women, are navigating this with layers that compound every barrier: medical racism, economic precarity, institutional distrust that is entirely earned given the history of how healthcare has treated Black and Brown communities, and a political environment that is hostile in ways that don't feel abstract because they aren't. The people most targeted by these laws are often the people who have had the fewest safe options to begin with.
Blue Sage Wellness was built for exactly this moment, not in reaction to it, as a long-standing commitment. Our founder, Dr. Chanel Thomas, is a Black, queer, neurodivergent psychiatric nurse practitioner who created this practice because her communities deserved care that doesn't gaslight, dismiss, or disappear when things get hard. What GAHT Support Looks Like at Blue Sage Wellness
Psychiatric evaluation for GAHT: A comprehensive appointment to understand your mental health history, your goals, and what support would look like going forward. This is affirming, collaborative, and thorough. It is not a gatekeeping process.
Letter writing: If you need documentation for a surgical referral or another purpose, that's something we can discuss as part of your care at BSW.
Ongoing mental health support: Starting or continuing hormone therapy brings up a lot: changes in relationships, body, identity, and sometimes grief alongside joy. We're here for that part too, not just the evaluation.
Collaborative care: If you're already working with an endocrinologist or primary care provider, we can coordinate with them. You shouldn't have to manage your care team alone.

Is gender-affirming hormone therapy legal for adults in Tennessee?

Do I need a referral to start?
